I confirm that my son/daughter has an active private medical insurance policy. If not, I also confirm that my child is covered by the institutional insurance provided by their school or the organization hosting the event.

I authorize my child to participate in all activities organized by Prezpower American, including games, music, group dynamics, fencing, puppetry, and other educational, artistic, and sports experiences.

Optionally, I may request the temporary extra coverage offered by DG | La Isla de los Niños for this event. I accept the previously informed additional cost and understand that this insurance is complementary.

I declare that I have read and agree that all payments made are final, non-refundable, and non-transferable under any circumstances.

I understand that this additional coverage does not replace the child’s private or institutional medical insurance. The price of this coverage may vary depending on the event or the age of the participant.

I have read and accept the Waiver of Liability described below. I understand that this document is valid from the moment my child attends classes, training sessions, or tournaments organized by Prezpower Fencing Club / Prezpower After School / Art+ Sport+ Values Program, whether or not the registration form has been completed and whether or not payment has been made.
I hereby acknowledge and accept that fencing, like any sport, involves inherent risks of physical injury, accidents, or personal harm. I understand that despite all safety precautions, accidents may occur during practice, training, tournaments, and any club-related activity.
I voluntarily assume all risks associated with participating in activities of Prezpower Fencing Club / Prezpower After School / Art+ Sport+ Values Program. I expressly release and hold harmless Prezpower Fencing Club / Prezpower After School / Art+ Sport+ Values Program, its coaches, staff, and volunteers from any and all liability for any accident, injury, damage, or loss that may occur during practice, the use of borrowed or personal equipment, or any activity organized by the club, including transportation to or from the event location.
I declare that, as parent/guardian, it is my responsibility to ensure that my child has undergone a sports physical check-up before starting and at any time during participation. It is also my responsibility to maintain valid medical insurance with USA Fencing and to ensure that my child has the complete uniform as soon as possible.
I agree to follow all safety rules and to use the required protective equipment. I understand that it is my responsibility to ensure my child uses protective gear.
By checking this box and/or signing this document, I agree not to bring any legal claims against Prezpower Fencing Club / Prezpower After School / Art+ Sport+ Values Program or its representatives for any injury or damage suffered in connection with the above-mentioned activities.
